The reason that violations of the LOTO Standard are among the Top 5 most frequently cited Standards every year is that employers must self-report work-related amputations and admissions to the hospital within 24 hours. As a consequence, it is likely that such cases often involve an employee's failure to lock out or the employee's decision to reach past guarding into a machine while the machine is still running.

These cases often result in OSHA conducting an on-site inspection and ultimately result in citations alleging violations of the LOTO standard. It is, therefore, crucial to understand how OSHA interprets an employer's compliance obligations under the LOTO Standard.

He works with companies on compliance with both federal Occupational Safety and Health Administration and state safety and health regulations.
